Hong Kong East Cluster has a long-standing commitment to promoting Medical Social Collaboration and integrating it into patient journeys. By leveraging community resources, we aim to strengthen the connection between hospitals and community services, providing seamless holistic care for patients.
Since 2005, Hong Kong East Cluster has recognized the importance of the close relationship between medical and community care in patient rehabilitation in the community. To this end, the Cluster established the Community Services Office and set up 7 cluster-based communication platforms and 11 panels and working groups. These platforms and groups not only promote communication among different medical teams but also develop different collaborative models using various strategies.
Through many years of efforts, Hong Kong East Cluster has now partnered with over 300 community partners, including non-governmental organizations, patient associations, local bodies, and government departments. These partnerships form a stronger team for holistic care, emphasizing the importance of collaboration and community engagement in healthcare.
